Crisp Variables in Fuzzy Model

Hello there!

I m writing my tese and biulding a fuzzy model for predict cervix cancer. In my theory i have fuzzy variables, like age for exemplo, but i also have crisp variables, smoker for example (the answer is yes or no), anybody knows how can i make this variable and interact with the model? i tried to find a whay to creat a simples variable but it has to take a value and range in the x axis.
