Templates/Examples for Design Logs

Does anyone have a template/examples of Design Logs for events like Flight or Wind Power?

The Flight log is much simpler this year, back to what we've had in the past, nothing like last years overdone log.
A simple table with six columns for the parameters and 10 rows for the flights, properly labeled is all you need.  Can be handwritten.
Of course if your teams to be successful that isn't even a bare minimum, but it is all you need to meet the requirement.
Note, the parameters you pick (beyond the three minimum required) are dependant on your teams specific design as to what can be adjusted.
Here is an example I use in my Flight presentation used at the SO summer clinic, MI state clinic and other places.  It is tailored to the designs my teams were using that year, so modify parameters as needed. Oh, and of course it should have units, easiest way to do this would be to add them to column headings. Team name and number also good.
