If any of you out there in cyberland have had a vasectomy
and then developed acute and chronic prostatis or
epididymitis your treatment needs to be handled alot
differently. Your Dr. will probably not recognize the
corelation between the two for medicolegal reasons. It can be
an awful condition. It is called A Late Post Vasectomy
Syndrome. Journal of Urology March 1985 Vol. 134 Drs.
Selkowitz and Schned. The truth is out there!

I don't have access to those journals. Possible to post crucial extract
here, please?

Prostatitis post vasectomy and post vasectomy pain syndrome are two
distinct entities with different causes and treatments. Please do not
confuse them as it may lead you down a stray path.

I guess that you have suffered from Post Vasectomy Syndrome
that includes epididymitis and protatitis? No you just are
the good Dr. telling it from your side of the table. Too bad
you really don't understand the condition or how to resolve
it. We'll call you the Spin Doctor, Doctor. At least you did
not deny its existence like most do. I'll give you credit
for that.
I've been there and suffered for 9 yrs. I am well versed
on this condition.

Dr. Werthman
Could you explain "Prostatitis post vasectomy"? I have never seen
anything definitive linking vasectomy with CP, but have often thought
it might be interesting to take poll on the number of CP patients who
have also had a vasectomy.

Out of curiousity, I passed by the local Medical Library on my way home
and looked this up. By the way, "SWOOSH", your reference was
incorrect. It WAS in volume 134, but September, not march. Very
interesting.. I will quote some of the paragraphs:
.
ABSTRACT: "A group of 20 surgical specimens in 18 patients with a
previously unappreciated syndrome of unremitting epididymal pain and
induration 5 to 7 years after vasectomy was collected during a 2-year
interval. These symptoms uniformly were unresponsive to conservative
measures, including empiric antibiotics. Total unilateral or bilateral
(1-side or both sides) epididymectomy and partial vasectomy led to
complete relief of symptoms, usually within 24 hours. Pathological
examination of the specimens revealed features consistent with sequelae
of long-standing obstruction. Recognition of this late post-vasectomy
syndrome, which represents a major complication of vasectomy, might be
expected to increase as cohorts of vasectomized individuals age."
.
"of 1,000 patients studied by Klapproth & Young, 5 percent had early
post-vasectomy complications, and 1.3 percent had late post-vasectomy
complications.". (your chance of getting the LATE post-vasectomy
complication is slightly more than 1 in 100.) "Unfortunately, many of
these patients had been treated for infectious epididymitis for
protracted periods because the true nature of the disorder went
unrecognized".
.
""The epididymis and the attached vas deferens were removed as a single
specimen all the way to the previous vasectomy site. The testis, with
its vascular supply was preserved in all but 1 case."
.
"the most commony encountered histological feature in the afflicted
epididymis was dilation of the tubules, which usually was of mild to
moderate degree and focal in nature, with clusters of dilated tubles.
Occasionally, the dilation was pronounced, even cystic, measuring up to
6mm in diameter. Dilated and nondilated tubules often were packed with
spermatozoa and histiocytes."
.
"All patients had been subjected to an empiric trial of antibiotics, and
anti-epididymitis measures, such as sitz baths and scrotal supports, for
at least 6 weeks without success. Patient discomfort usually was
constant, often disabling, exacerbated by sexual activity, and
occasionally radiated along the spermatic cord structures"
.
"Since the cure of this problem is surgical and immediate, and because
the vasectomy procedure is so prone to medicolegal complications, we
believe that the late post-vasectomy syndrome should be included in the
informed consent form for vasectomy."

I find it very discouraging when the posts are met with this kind of
sarcasm. Maybe it is my fault for not further defining the condition. I
realize that you are suffering but I am trying as best I can to make sure a
destinction is drawn between often confused entities so patients can find
the appropriate treatment.
Very simply put, the post vasectomy pain syndrome is congestion and
pressure in the epididymis caused by obstruction. It can be resolved by
reversing the vasectomy and relieving the pressure.
This is very different than pain following a vasectomy because of a variety
of other reasons such as an infection, epididymitis, prostatitis,
entrapment of a nerve, granuloma formation, ect. Clearly the treatment for
these problems is very different than that of post-vasectomy pain syndrome.
Surgery should not be done to cure these other conditions.
I am not trying to put a "spin" on anything, all I want is for patients to
understand the condition so they can be better educated and seek
appropriate help.

Your just wrong about how to treat is. Reversal is out my
friend, it doesn't work it just adds $$$ to the doctors who
then hace to do an epididyecytomy since there is already a
blown epididymis. I should know I've had both done to me .
You need to talk to Selkowitz. Its a bitch when a poor
schmuck knows a little bit more than a pompus MD.
Sooner or later a Urologists is gonna get it from screwing
up someone's life over a vasectomy. The truth has to be to
be put on all CONSENT FORMS.
