diagnoses please

Over twelve years ago I had a vasectomy. I have had problems with pain and
swelling in my testicles ever since then. Usually the pain was not bad
enough to see a doctor. Seven years ago I had a period of six months of
severe pain which eventually subsided. Last year the pain returned. A
long course of antibiotics did no good.

I had a vasovasostomy. The right testicle had a lot of back-up pressure
and green fluid. The vasovasostomy gave me a couple weeks of relief, then
the pain returned.

I then had a right inguinal orchiectomy (the pain has always been worse on
the right side.) I felt great. I gradually resumed my activities,
walking, working out with weights at the gym, etc. always being very
careful and conservative. Two months after my operation I tried increasing
the intensity level of my workouts. Now the pain has returned, mostly on
my left side, but sometimes on my right side. The pain came approximately
twenty-four hours after working out. I have not been to the gym since then
(two weeks ago) but the pain has only gotten worse.

I have had blood work and urinalysis done. They showed nothing wrong. No
infection.

Everything functions normally: urination, intercourse, etc. No additional
pain during these activities.

I have had sonograms of my testes, prostate, seminal vesicles, etc. The
showed nothing wrong. Only moderate swelling in the testes.
