A 39 year old woman, 7 months pregnant, 3d pregnancy (the first 2
uneventful) consults a gynecologist because she has had an important
discharge of a water-like liquid on the morning of the consultation and the
night before.

Observations: polyuria, the uterus is about 2 cm, not dilated. The patient
feels dizzy and is worried.

What question(s) one must ask ?
What exam(s) one must conduct?

And , most importantly:

What decision(a) one must make?

This is a real case and your opinions, anonymous or not, are greatly
apreciated.

Thank you.

I think you should differentiate between loosing urine and premature rupture
of membranes if I do understand your (few) information correctly

Is this woman experciencing contractions?
Fever?
Bloodloss?
Incontinency for urine?

What do you mean with "uterus is about 2 cm"?

Does the size of the uterus match with the duration of pregnancy?

And what about the fetal well being? (ultrasound/fetal heart rate)

About feeling dizzy, ...anemia?
Diabetes? (dizzy in combination with polyuria)
