It looks like #2 (divide by 2.2).

Don't forget the units we are working with.

LDL=CHOL-HDL-VLDL; where VLDL is

CONVENTIONAL UNITS (MG/DL): VLDL = TRIG/5
SI UNITS (MMOL/L) VLDL = TRIG/2.2

Friedwald indicated that this equation in not appropriate if the Trigyceride is
greater than 400 MG/DL (4.52 MMOL/L) or on samples from patients who have type
III Hyperlipoprotinemia (ELECTROPHORESIS "BROAD BETA" LIPOPROTEIN) present.
