Input requested for CA Healthy Kids Survey assessment questions

Dear all,

We have a chance to add up to 6 multiple-choice questions to our district’s implementation of the California Healthy Kids Survey for all of our district’s 5th grade students. We are hoping to add meaningful questions that we’ll be able to track year after year. I’d like to ask you for your suggestions for what kinds of questions would be most helpful to include.

For background:

"The California Healthy Kids Survey (CHKS) is an anonymous, confidential survey of school climate and safety, student wellness, and youth resiliency. It is administered to students at grades five, seven, nine, and eleven. It enables schools and communities to collect and analyze data regarding local youth health risks and behaviors, school connectedness, school climate, protective factors, and school violence. The CHKS is part of a comprehensive data-driven decision-making process on improving school climate and student learning environment for overall school improvements."

The questions that we would like to add have to do with access to the outdoors and wellness/healthy eating/etc. Some kinds of questions that we have been considering adding (wording or multiple-choice answers might be changed) are:

Do you have lessons (not recess) in a school garden or outside on the school grounds?
No, never
Yes, some of the time (about one time each month)
Yes, most of the time (about one time each week)
Yes, all of the time (every day)

During the past month, on how many days did you spend an hour or more outside in a garden or in a natural spaces like parks that have trees and plants?

As a followup to my earlier post: in case anyone is curious, here are the 6 questions that we did add to our district’s CHKS for 5th graders.
