Sign-Up Sheets for School Garden Beds

Hello! I am wondering if anyone has any example sheets or forms they want to share (or just advice) for encouraging staff and families to sign up to take ownership of a garden bed or potentially "sponsor" or adopt a bed?  I occasionally have teachers ask for example sheets to use, and I am curious what already exists that others use or methods you all have used for organizing this. Thank you!
