CIRCULAR

This is to inform all concerned that, we are assigned following hospitals for COVID Vaccination (COVISHIELD) for members who have already registered for vaccination through HR department earlier in Dec 2020

MANDATORY DOCUMENTS (any one) TO CARRY IN ORIGINAL

All members are advised to carry with them the ID Proof they had submitted to HR department for COVID vaccination like PAN Card, Driving License, Election card, Bank Passbook only and not AADHAR CARD

Please also carry your ID Card issued by SCEH

VACCINATION DAYS

Monday, Tuesday, Thursday and Saturdays starting 30^th Jan 2021 (Saturday) from 9 am onwards

Every centre there is a limit of 100 vaccinations a day which would be on first come first serve basis

LIST OF MEMBERS

Department heads will assign their member’s the days for vaccination as per the COVID app list sent to the Health Authorities

Doctors are advised to inform Dr Arpan Gandhi one day before they are planning to go for vaccination so that we can finalise the list

HR department would be the coordinating the same

PLACE OF REPORTING

We shall be arranging vehicle for pick up and drop to the assigned facilities

Reporting at 08:20 am (Sharp) as per pre decided scheduled days at Gate no 1 with documents as mentioned above as per the list made

We would like to be there 10 minutes prior to the schedule time of 9 am

Members can also go directly to the above mentioned hospitals but should try report by 9 am

PROCESS TO BE FOLLOWED

Registration would be done followed by vaccination at the above hospitals

30-45 minutes of observation by the medical team of the hospital at their premises after vaccination is done

Then all member’s will come to SCEH so that in case of any issues observation / advise can be done

Information to Mr Harish Kumar / Mr. Amber / Mr. Prakash that their vaccination is done so that our records are updated through whatsapp / sms / email

In case anyone report any issues the can contact Dr. Nidhi Dhawan / Dr Arpan Gandhi / Ms. Alice / Mr. Harish Kumar(HR) immediately
