1.       Full premium chart for Staff and Executive of sum Insured of Rs. 3 lac, Rs.5 lac  and Rs.7 lac inclusive of GST.

 

2.       Policy terms & conditions

 

You all are requested to please share your declaration on or before 20^th Nov 2020.

 

Further please read each and every terms and conditions of this mediclaim policy before filling a form.

Hospitalization Health Insurance Scheme tie up with

New India Insurance Co. Ltd.

 

We are pleased to inform you that the Group Medical Insurance Policy for you and your family member for the period of 2020-2021 has been renewed with  New India Insurance Co. Ltd.  and its third party administrator viz. Raksha (TPA) at reasonable and affordable cost.

 

The hospital shall be paying Rs.1,000/- of the total Mediclaim premium payable by the employees drawing the Gross Salary up to Rs. 50,000/- (from its end).

 

All employees are hereby informed that the FORM (Attached) to apply for Mediclaim Policy is available with HR- Department:

The Last date to fill up the form is 20th November 2020.

 

We will also share premium chart after getting request for enrollment.
