Unable to start a new farm

Hi,

I have been using scalr for a few weeks and am quite happy with the
start and stop of the instances and farm.
We have 3 farms in total 1 running and 2 terminated.
Today when I tried to create a new farm with app, www and mysqllvm it
is not starting. It is going from state pending to initializing to
terminated and in a loop.

Does mysqlivm have known bugs while startup? I had no problem in
starting up app, www and mysql.
I terminated and deleted the farm and tried again. Again it is going
in a loop for last 30 minutes.

Could you please help me?
