All logs are full of "Instance 'i-ea453183' found in database but not
found on EC2. Crashed.".
All servers were up but Scalr started more instances. Oh my god.

I'm having same issue.
My instances were restarted 5 hours ago without any reason.
Is there any problem right now?
It seems I didn't do anything special except changing my password.

Same errors for me
Here are the logs (farmID 930)

Instance 'i-24c27e4d' found in database but not found on EC2. Crashed.
10.253.195.99 DOWN: Scalr notified me that 10.253.195.99 of role www
(Custom role: www, I'm first: 0) is down
Instance 'i-27c27e4e' found in database but not found on EC2. Crashed.
Disaster: No instances running in role www!
Starting new instance. InstanceID = i-a91e6fc0.
Disaster: No instances running in role app!
Starting new instance. InstanceID = i-ab1e6fc2.

How do I know if this is a scalr problem or an Amazon problem. Doesn't
seem to be any special issues with US instances on
http://status.aws.amazon.com

Also i can't run any instances on my second farm, few minutes instance
status is pending, and then terminating
seem like disaster

It was a user error on scalr.net. This post throws a bit more light:
