Duplicate order line items?

I just had an order come in that has duplicate line items for every product ordered. For example: there is two line items for product A, with quantity 3 on each line item... (see screenshot)

any ideas what in the world could of happened? This is a pretty busy store - tens of thousands of orders - first time I've seen this.
