water connection form

Dear All,

I just want to help those who cannot read and write telugu in furnishing the

individual water tap connection.

Page-1:

1) House owner name and address in BLOCK letters

2) D.No.

3) a) Street, locality, Ward No.: Sardar Nest, KBR Road, Ward No. 52

    b) White ratio card no.:

4) House Tax assessment No:

5) Tax being paid for half year:

6) tap connection for:  individual house/group house/apartment/.......  (so choose first option)

7) leave it

Page-2: Your name in the blank and signature at bottom

              Tick 1 and 2  (you are attaching house pass book and latest receipt for tax payment)

Page-3: first blank: owner's name

              second blank: Father/Husband name

              third blank: assessment no.

              fourth: D.No.

              fifth:  Street name

              sixth: Ward no.

              Sign at left bottom and two witness signatures.

All the simplex and duplex owner's have to furnish the form.

please find attached herewith the form. You have to provide supporting documents as mentioned and post the forms.

This is strictly for simplex and duplex only. 

For apartments the individual connection is not provided. Only semibulk or bulk connections are provided. Builder might have taken steps already.
