Custom Visualforce Page for Lead Convert page

Hi everyone,

I have a scenario......i have  to create a custom Visualforce where we have to mimic standard page with this page and add all the fields that are converting from Lead to Account, Contact and Opportunity for that we created a custom visualforce page for Lead Conversion page but i could not able edit TASK information and Description Information section fields and i could not able to get lookup for Record Owner and Opportunity name fields.

can anyone have implemented this scenario or suggest me the code to accomplish this scenario.

for creating a lookup field in a vf page we need to write a controller or an extension which captures the id of the object and assigns it to the field
