Can you tell me if you add a logo when you are just using the basic version? I have created a form from sctratch rather than editing a template.

Hi K,

You need to add a new field; perhaps name it Logo.

Goto Label and lines button.

Select Background Image for type.

Select your image.

Hope this gets you going.
