Code review request: porting remaining models to the multigpu branch

Hi everyone,

I've just opened a pull request in our GitHub repository:

https://github.com/sailfish-team/sailfish/pull/8

and would appreciate it if somebody could review the code.

The pull request contains:
- changes to port the single fluid Shan-Chen model and the free surface model
to the multigpu architecture,
- utility functions to automatically merge partial results from subdomains,
- various fixes in the whole codebase.
