A self-admitted fan of DOCTOR WHO, he persuaded writer Andrew Cartmel to set up an introduction, which led to an association that, to date, has included music for the DOCTOR WHO spin-offs ROSE TYLER, MISSY, ROBOTS, and THE PATERNOSTER GANG. Other series have included the original psychological thriller TRANSFERENCE, three THUNDERBIRDS adventures, and FIRST ACTION BUREAU, also created by Gerry Anderson.

The number of records was reduced by aggregating records with identical values for these seven variables and adding a count variable to the aggregate record indicating the number of identical records.
Users of the CMF 1999-present on CDC WONDER can obtain death rates and counts (and maps and charts) for counties grouped according to a six-level urban-rural classification scheme. The most urban category in the scheme consists of central counties in metropolitan areas of 1 million or more and the most rural category consists of non-metropolitan counties with no urban clusters of 10,000 or more population. Methods and results: This retrospective study used National Vital Statistics Data from 1999 to 2016 provided by the Centers for Disease Control and International Classification of Diseases (10th edition) codes for heart failure and dementia as defined by the Medicare Chronic Conditions Data Warehouse. However, under the age of 65 years, higher crude mortality rates (per 1,000,000 person-years) were reported in men (0.20, 95% CI 0.18-0.22) compared with women (0.15, 95% CI 0.13-0.16; P < .001).
