จากที่สอบสวน พนักงานขับรถไม่ได้ลงมาจาตัวรถ พนักงานเพียงแค่เปิดประตู เตรียมจะลงจากรถ ขณะนั้น รถก็เคลื่อนตัวไปด้านหน้าไปชนกับเสาแบริเออร์ ดังกล่าว

ขอทางพี่ review กบทางเซฟตี้ แล้วทำสรุปเป็น Action ที่เราคุยกันในห้องประชุมตามหัวข้อด้านล่าง และจัดทำเข้าแบบฟอร์ม และแจ้งให้ทราบอีกครั้งค่ะ
- จัดทำขั้นตอน การจอดรถ EV ทุกประเภท ส่งให้ทาง DHL อนุมัติก่อน => ส่งภายในวันศุกร์ที่ 14 มิ.ย. 67 ช่วงเช้าก่อน 10:00น. ซึ่งทาง TPT Quality จะทำการตรวจสอบ แล้ว confirm กลับ ภายในเวลา 15:00น.
- ทาง SKY X นำขั้นตอนการจอดรถ EV อบรมพนักงานขับรถ EV ทุกนาย แล้วส่งหลักฐานการลงชื่อเข้าอบรม พร้อมรูปถ่ายประกอบ ให้กับทาง TPT Quality ภายในวันจันทร์ที่ 17 มิ.ย. 67
- ติดตาม การปฎิบัติงาน ของพนักงานขับรถ ชื่อ ถาวร ระยะเวลา 1 เดือน โดย DHL TPT Quality จะทำการสุ่มของหลักฐานการตรวจติดตาม
- การซ่อม แท่นแบริเออ ทาง SKY X จะดำเนินการซ่อม ใหห้แล้วเสร็จ ภายในวันที่ 24 มิ.ย. 67

คือการจอดรถไม่ดับเครื่อง (สำหรับรถ EV คือการไม่ปิดสวิตท์กุญแจ ) การเข้าเกียร์ทิ้งไว้ และไม่ดึงเบรคมือ = ให้ระบุในสาเหตุรายงานอุบัติเหตุ
โดยผมได้แนบรายงานอุบัติเหตุ, = ไม่พบเอกสาร
รายงานการตรวจเช็ครถรายวัน = เอกสารเป็นของเดือนอะไร

จากการสอบสวนอุบัติเหตุ กรณีที่ รถ EV 3ฒส-6787 กทม.ชนแผงกั้น บริเวณ ด้านข้างตู้ชาร์ต บริเวณ DHL W/H1 จนทำให้เกิดความเสียหายนั้น ซึ่ง นาย ถาวร หริ่งทอง เป็นผู้ขับขี่ สรุปใจความได้ว่าพนักงานท่านดังกล่าวไม่ได้ปฏิบัติตามขั้นตอน การขับขี่เชิงป้องกันอุบัติเหตุ จนทำให้เกิดขึ้นความเสียหายเกิดขึ้น คือการจอดรถไม่ดับเครื่อง (สำหรับรถ EV คือการไม่ปิดสวิตท์กุญแจ ) การเข้าเกียร์ทิ้งไว้ และไม่ดึงเบรคมือ ทางบริษัทฯ ได้เรียกพนักงานท่านดังกล่าวมาตักเตือนพร้อมได้ชี้แจงให้พนักงานท่านดังกล่าวได้ทราบถึงเหตุที่เกิดขึ้นชึ่งเกิดจากการไม่ปฏิบัติตาม ขั้นตอนการขับขี่ที่ปลอดภัย พร้อมออกหนังสือเตือน กับพนักงานท่านดังกล่าว เพื่อให้ตระหนักถึงความสำคัญของการขับขี่เชิงป้องกันอุบัติเหตุ
แนวทางแก้ไข ทางบริษัทฯ จะะจัดทำขั้นตอนการจอดรถ สำหรับรถ EV ติดไว้กับรถเพือเป็นคู่มือในการใช้งาน สำหรับพนักงานท่านดังกล่าวคือ นายถาวร หริ่งทอง ซึ่งเป็นพนักงานที่ดีและมีความตั้งใจในการทำงานเป็นอย่างดี แต่ผิดพลาดด้วยความ ชล่าใจ กับความปลอดภัยในการขับขี่ ทางบริษัทฯ จะทำการอบรมเน้นย้ำ และควบคุม พฤติกรรมในการขับขี่เป็นเวลา 2 สัปดาห์ นับจากนี้ พร้อมรายงานให้กับทาง DHL ทุกวัน
โดยผมได้แนบรายงานอุบัติเหตุ, หนังสือเตือนพนักงาน, รายงานการตรวจเช็ครถรายวัน, รายงานผลตรวจสอบเสพติดและแอลกอฮอล์และใบบันทึกความเสียหายครับ
