After rediscovering this, I think I remember that when you manually created a shortcut in Program Manager, the icons embedded in progman.exe were provided as default choices, or you could click a Browse button to open another .ico or .dll file like moricons.dll. But it's been so long ago, I don't have a copy to play with to confirm, and haven't been able to find information about it online.

Like most people above I thought it wasn't working either, but it turned out my image size was too big, you have to resize to 32px x 32px or less before you can save as an .ico format.. I personally put the plugin here :- C:\Program Files\Adobe\Adobe Photoshop CS5.1 (64 Bit)\Plug-ins\File Formats.

Images that are used as icons in the Windows operating system must be saved in ICO format. By default, Adobe's Photoshop image-editing program does not support ICO format, making it impossible to make an ICO file in Photoshop on a Mac computer with the default configuration. Using a file format plugin, however, Photoshop users can create ICO files on a Mac.

For the plugin to function properly it must be installed to correct folder. Double-click the plugin's DMG file to mount the image and drag the plugin file onto the computer's desktop. Click the "Finder" icon in the computer's dock, followed by the "Applications" folder icon. Click the "Adobe Photoshop" folder, followed by the "Plug-ins" folder. Drag the plugin file into the "File Formats" folder inside the "Plug-ins" folder.

With the plugin installed, you are ready to create an ICO file. Click the "Finder" icon in the computer's dock, followed by the "Applications" folder. Double-click the "Adobe Photoshop" application icon to launch the application. Open or create the image that you want to save in ICO format, click the "File" menu and select the "Save" option. Click the arrow to the right of the "File Format" heading and select the "ICO (Windows Icon)" option. Click the "Save" button to create the file.

If you are having trouble using the plugin, you must first check that it is properly installed. Click the "Photoshop" menu and move your mouse cursor over the "About Plug-In" option. If "ICO (Windows Icon)..." is listed in the side menu that appears, the plugin is installed properly. If it is not listed, quit Photoshop and repeat the installation procedure. Also, you cannot save an image that is larger than 256-pixels tall or wide in ICO format. If the image is larger than this specified width or height, the ICO format will not appear in the "File Format" drop-down menu.
