From the numbers of opportunities I've seen, definitely the USA, but
that's partly because we have so many big cities.
Getting more granular and asking about cities, it seems San Francisco
is the mecca, so to speak, followed (distantly) by New York, Chicago,
and Washington DC. Outside the USA I've seen a large number of
postings for London, probably somewhere between the numbers for NY and
DC. Other than that, the only ones I can recall seeing any
significant numbers for are Berlin, Rotterdam, and Amsterdam.
Hop on the major job boards, search for Ruby on Rails, and see what
areas pop up most often....

I also know of recent jobs openings in Christchurch (if you can't take the shake), Nelson, and Auckland.
I would suspect the market in Australia is hot. Sydney, Melbourne, and Brisbane probably best bets, though supposedly there is a big shortage of IT workers (haven't heard Rails specifically) out west to support the mining industry.
Australian pay is better than NZ pay due to exchange rates.
Personally, I'm looking for at least one junior level developer to work with me on projects as a freelancer. Remote from anywhere is fine as long as you have excellent English language communication skills. If you can't write and speak in English well, it's a non-starter. Oh of course, how could I forget -- last year, Puppet Labs was trying
to convince me to consider moving to Portland! Very nice city
(visited there last summer), but I don't want to move. :-) New
Relic's also there, and a fair assortment of startups. Still fewer
than DC-land, what with all the federal (and federal contracting)
jobs, tho. I've seen some for Austin too, but not enough to make an
impression.
