Dear President / Secretary,

This is further to my letter dated 27.08.2012 seeking Nominations for District Governor Nominee 2015-16 from the clubs of R I District 3190 as per the Pilot.

Please find attached Profile of Candidate for DGN 2015-16 which is required to enable nominating committee evaluate the candidate.

Completely filled profile should be submitted in 4 Pages only of A 4 size, and should not have any photographs other than One Picture of the Candidate and Spouse.

Nominations for District Governor Nominee 2015-16 proposals with 15 copies each of FORM H and Profile of Candidate along with the certified resolution should reach my office by 27nd OCTOBER 2012 by 5.00 pm. Any Nominations received thereafter shall not be considered.
