It has been brought to my attention that the CMU speech vocabulary tool used in the book is currently unavailable since the host seems to be down. The tool is used in the book in Chapter 9.3 and is linked to at http://www.speech.cs.cmu.edu/tools/lmtool-new.html. Since I have no control over the CMU servers, I'm not sure when it will be up again and there do not appear to be any mirror sites. In the meantime, I am trying to figure out if the source can be downloaded locally and run on your own machine. However, the key Perl script is also unavailable since it is stored on the same CMU host.
I'll keep you posted.

Looks like the CMU speech site is back up.
