In February 2016, during our previous comprehensive inspection of Dr Ajaz Nabi (also known locally as Cippenham surgery), we found issues relating to the effective delivery of healthcare services at this practice. As a result of this inspection, we asked the practice to make further improvements, in order to encourage the uptake of health promotion services offered at this practice. Health promotion is a process which a practice can use to help patients improve their health and wellbeing. Health promotion can involve a range of activities, from encouraging and advertising flu vaccines, to promoting the importance of cancer screening.

As a result of these issues, the practice was previously rated as requiring improvement in effective services, and good for safe, caring, responsive and well led services. This meant that the practice had an overall rating of good.

We carried out a desk based inspection in October 2016 to ensure the practice had made improvements since our last inspection. The practice sent us evidence in the form of letters to patients, patient information leaflets in a number of different languages, and health posters to demonstrate the wide range of improvements they had made since our last visit. The practice also further supplied an audit schedule; this outlined the different audits which were carried out in the practice and how often each audit should take place. Finally the practice was able to provide further supporting evidence, in the form of statistical figures, to demonstrate the impact that changes made to their patient population. We found the practice had made significant improvements since our last inspection in February 2016.

Following this desk based inspection we rated the practice as good for providing effective services. The overall rating for the practice remains good. We carried out an announced comprehensive inspection at Dr Ajaz Nabi (also known locally as Cippenham surgery), 261 Bath Road, Slough, SL1 5PP on 3 February 2016. Overall the practice is rated as good.

Importance: Expensive technologies-including robotic surgery-experience rapid adoption without evidence of superior outcomes. Although previous studies have examined perioperative outcomes and costs, differences in out-of-pocket costs for patients undergoing robotic surgery are not well understood.

Design, setting, and participants: A retrospective, cross-sectional, propensity score-weighted analysis was performed using deidentified insurance claims for 1.9 million enrollees from the MarketScan database from January 1, 2012, to December 31, 2017. The final study sample comprised 15 893 US adults aged 18 to 64 years who were enrolled in an employer-sponsored health plan. Patients underwent either an open or robotic radical prostatectomy, hysterectomy, partial colectomy, radical nephrectomy, or partial nephrectomy for a solid-organ malignant neoplasm. Statistical analysis was performed from December 18, 2018, to June 5, 2019.

Main outcomes and measures: The primary outcome of interest was out-of-pocket costs associated with robotic and open surgery. The secondary outcome of interest was associated total payments.

Conclusions and relevance: This study found significant variation in perioperative costs according to surgical technique for both patients (out-of-pocket costs) and payers (total payments); the robotic approach was associated with lower out-of-pocket costs for all studied oncologic procedures.

Dr Nabi completed his general surgical training in South Australia predominantly based through the Royal Adelaide Hospital and Flinders Medical Centre and then completed further colorectal specific post-fellowship training in Adelaide, Sydney (Bankstown and St George Private Hospitals) and Perth (Fremantle and St John of God Hospitals).

Dr Nabi has public appointments at the Mater Adult Hospital and Logan Hospital and is also employed by the University of Queensland as a senior lecturer. He has diverse research interests which include varied aspects of colorectal surgery and surgical education for which he has been widely published and presented at national and international meetings.

For many leaders, the common perception is that provision of surgical care is not sustainable or cost-effective. When faced with tough budget choices, governments often favor programs that combat infectious and chronic diseases, leaving people like my rickshaw driver in the lurch.

But a growing body of evidence suggests that these views are misplaced. When researchers at the Harvard Medical School analyzed surgical interventions in low- and middle-income countries, they found a remarkable disconect between economic assumptions and reality. Longer-term strategies include investing more in medical education to bolster the ranks of doctors and surgeons.

Dr Hajir Nabi is a General Surgeon consulting and operating at North West Private Hospital and St Andrew's Hospital. Dr Nabi has diverse interests in all aspects of colon and rectal surgery including colorectal cancers (and polyps), diverticular disease, inflammatory bowel disease, functional bowel disorders (including rectal prolapse) and anorectal conditions (haemorrhoids, fissures and fistulas). He is an experienced endoscopist offering both colonoscopy and gastroscopy (with GESA accreditaton).

Dr Nabi completed his primary and secondary schooling in Adelaide, South Australia. He then went on to complete his medical degree (MBBS) through the University of Adelaide finishing in 2004. His general surgical training was undertaken predominantly through the Royal Adelaide Hospital and Flinders Medical Centre in South Australia. Dr Nabi completed his general surgical examinations in 2012 and was awarded Fellowship of Royal Australasian College of Surgeons (FRACS).

Having developed a keen interest in colorectal disorders and their management during his general surgical training he undertook post fellowhip training in colorectal surgery through the CSSANZ training program. This incorporated 2 years of subspecialty training in Sydney (Bankstown and St George Hospitals) and Perth (Fremantle and St John of God Hospitals).After accomplishing additional colorectal specific research and examinations he was awarded membership of the Colorectal Surgical Society of Australia and New Zealand (CSSANZ). He has numerous research publications in international journals focusing on colorectal surgery and surgical education, and has presented his research at national and international meetings.
