Grant Forms contains copies of the federal forms currently used by awarding agencies to create grant application packages in Grants.gov, forms for post-award reporting, and forms that have been retired from the system. These include both government-wide and agency-specific forms.

The Standard Forms 424 (SF-424) Form Families represent the government-wide standard data sets and forms for grant application packages, which were developed in partnership with federal grant-making agencies and the applicant community. For more details about creating Grants.gov forms, please review the Forms Request page.

If you are applying for a SAMHSA grant, you must download the application forms found on Grants.gov. You may require additional forms found on the SAMHSA website. Read the Notice of Funding Opportunity (NOFO) carefully to determine which forms you must include in your application. Applications that do not include all required forms will be screened out and will not be eligible for review.

These are forms related to Government Accounting Standards (GAS) and the requirements for an adequate accounting system, general audit types applied to Federal awards, and a sample format for the development of indirect cost billing rates. They are used as part of EERE's financial oversight process and include:

The forms listed below are shown for information purposes only. For most forms and programs, the Department receives actual data electronically through systems (e.g., G5; grants. gov) in which it participates. Questions about the use(s) of forms in a particular program should be directed to the contact person for that program, found by clicking here or using the 'Search' field, above.

The program provides guaranteed loan financing and grant funding to agricultural producers and rural small businesses for renewable energy systems or to make energy efficiency improvements. Agricultural producers may also apply for new energy efficient equipment and new system loans for agricultural production and processing.

To complete an application for this program, you must be pre-registered with the System for Award Management (SAM) and also have a Unique Entity ID. Neither of these applications cost money, but they can take time so be sure to get this taken care of right away. If you are already registered with these systems, you do not need to do it again.

Applicants are encouraged to refer to Review Criteria of the Technical Review Panel [ download in English Español Français ] (TRP) and to the Review Approaches Manual [ download in English ] while developing their funding requests, to learn more about what the TRP will be looking for during their review.

An example funding request [ download in English Español Français ] has been developed to support applicants in their development of funding requests to the Global Fund. It demonstrates how applicants can structure their responses to the questions and prompts described in the Application Forms and Instructions below.

Always follow your funding opportunity's instructions for application format. Although these applications demonstrate good grantsmanship, time has passed since these grantees applied. The samples may not reflect the latest format or rules. NIAID posts new samples periodically.

The text of these applications is copyrighted. Awardees provided express permission for NIAID to post these grant applications and summary statements for educational purposes. Awardees allow you to use the material (e.g., data, writing, graphics) they shared in these applications for nonprofit educational purposes only, provided the material remains unchanged and the principal investigators, awardee organizations, and NIH NIAID are credited.

Freedom of Information Act (FOIA). NIAID is strongly committed to protecting the integrity and confidentiality of the peer review process. When NIH responds to FOIA requests for grant applications and summary statements, the material will be subject to FOIA exemptions and include substantial redactions. NIH must protect all confidential commercial or financial information, reviewer comments and deliberations, and personal privacy information.

The R21/R33 supports a two-phased award without a break in funding. It begins with the R21 phase for milestone-driven exploratory or feasibility studies with a possible transition to the R33 phase for expanded development. Transition to the second phase depends on several factors, including the achievement of negotiated milestones.

*Dr. Mark Poritz submitted the original grant application. In the course of the first year of funding, Dr. Andrew Hemmert took on increasing responsibility for the work. For the grant renewal, Dr. Poritz proposed that Dr. Hemmert replace him as the PI.

First, the Georgia home study parent/guardian official must create a GAfutures Education Professional account for the name of the home study program. The Georgia Home Study Program parent/guardian will receive an assigned HSP#, to provide to the student prior to completing the annual online Dual Enrollment funding application.

The funding application for Dual Enrollment must be completed by the high school and postsecondary institution no later than the last day of the postsecondary term, semester or quarter or the student's withdrawal date whichever occurs first.

As of April 4th 2022, eBRAP has transitioned from use of DUNS # to the UEI # issued by Sam.gov. In order for eBRAP to process any applications submitted through Grants.gov, the UEI must be entered into the Organizational Profile.

Routine Uses: The information requested may be disclosed to qualified reviewers and staff assistants aspart of the proposal review process, funding decisions, or the administration of research awards; or to othergovernment agencies for comparison of funded research. eBRAP does not share information with any privateorganizations and does not collect information for commercial marketing.

Please note that the forms should be downloaded to your computer BEFORE they are filled out, and not opened in a browser, to ensure proper functionality. Also, Word's macro security setting should be set to "Medium", as many forms may not work properly if Word's macro security setting is set to "High." To change this, launch Microsoft Word and go to: Tools -> Options -> Security. Click the "Macro Security" button, and choose "Medium".

Proposals where community engagement (defined in Box 1 in this document) is indicated must include a Community Engagement Plan. This plan should describe engagement with community groups and other end users of the work that have an interest in the proposed research. Community engagement has many different meanings and can be conducted through a variety of actions to broaden the impact of studies while simultaneously benefiting the communities involved. The level and type of community engagement should be appropriate for the proposed study. Cross reference your Project Plan (Form F-6) to indicate when and how you will engage with communities, explain how the engagement will contribute to achieving research objectives, and ensure that your budget (forms F-4 and F-5) reflects proposed community engagement activities. The community engagement plan may not exceed two pages.
