Missing elements in reportlab pdfs

I’m having an intermittent problem with PDF generation. Sometimes elements appear to be missing.

On some occasions the pdf build will fail with ‘list index out of range’. On others the PDF will build but an element will be missing. On others the PDF will build completely. This can happen on successive attempts to build the same PDF. I’m using reportlab 4.0.4 with python 3.11.
