No submit button?

I cannot for the life of me find how to get the submit button to
appear on a review.

I've +2'd it, +1 verified, but I see no sign of the submit button.

Any suggestions on what I might be missing would be appreciated.

check that you have submit privileges [1] and that there is no veto
voting (-2 in code review or -1 in verified) preventing submit

I'm in as an Administrator and I did specifically add the submit
access privilege to the Administrators group.

There are no negative scores.

Is there anything else? I'm pushing to HEAD:refs/for/master (IIRC),
it's not that I have to push to a different branch or anything like
that is it?

Double check and make sure your "Reference name" for the Submit is "refs/heads/*" . I have seen people mistakenly place "refs/head/*" instead of heads.
