Information being added and manipulated

My ex, an only child, and his father had the same name except his father had no middle name. My ex told me they were trying to comingle their information so when his father died, he would be able to continue receiving benefits, it would look like property was his and a host of other things. 

His mother was not in the greatest of health and was in a nursing home - not the nicest one. His father was a veteran and had been gassed so his breathing was bad. While we were together, his father went into the hospital and the family was called - we thought this was the end. My ex was distraught - how was he to live?  His aunt told him there was a large trust fund for him when his parents passed because no one in the family could imagine him being able to take care of himself. He calmed right down and was fine until his father got better. I had never seen him so angry. The day his father was to come home from rehab, he refused to go get him. I had to pick him up and bring him home. My ex refused to come out of the room to see him. 

Several months later, my ex suddenly announced he had been cheating on me and it was over. The person he was with is much older than his father, almost old enough to be his grandfather. Come to find out, this person's partner had mysteriously died earlier in the year, and he had moved about 470 miles - several states away - the day after the guy died. 

About a year ago, my ex's his mom died. 2 weeks later, the husband of his best friend died. Two weeks after that, my ex's father died.  None were in the greatest of health, but none near death. 

Now all the public records search sites have changes to the information about my ex, his father, his mother, his live-in grandpa. For instance, they are now showing his father having a middle name. He did not have a middle name. 

They have also had changes to my information. At first, I was completely gone. I had applied for a certain kind of Facebook account, and they required I get a notary to verify my ID and address because THEY could not find me. My information slowly started reappearing but with weird changes and inconsistencies. My name is not that common, and I am the only person in the US with my complete name combination, and there are about 40 people with my first/last combo. There are now two of me listed. One is an old address in another state that shows as current and information that had been there is gone. My current location is very incomplete and does not have the info it should. Weirdly, some of my information (siblings and some other such info) has shown up under a person with my first/last combo in the state where the grandpa moved from.

How is such manipulation of information occur? How do I stop it?
