University Scholarship Applications
This form is to be used by students to authorize consent for the Office of Student Financial Assistance to disclose federal tax information from their Free Application for Federal Student Aid (FAFSA) to the parties identified by the student.
This form must be submitted (with all supporting documentation) by a student who has been selected for verification on their FAFSA. The parent will also need to sign off on the V1 or V5 dependent worksheet, so if you have not yet received an update on a worksheet you submitted, have the parent check their email to make sure they have completed all their steps as well.

A student who is unable to appear in person must go to a notary public and sign the Statement of Educational Purpose. Students may use the Unusual Enrollment History Appeal form to request a re-evaluation of the termination of their financial aid.
