PageGUID Missing, error on Edit via Form reddot

Hello,

Has anyone experienced an error where instead of loading the form elements when a user clicks the "F" (edit via form) red dot they get an error that says PageGUID Missing, with nothing but an OK button to click on which closes the window? It's driving my editors crazy, and I'm not sure what's causing the problem. We have 2 servers in a clustered environment running Management Server 9 Build 9.0.1.76. I've tried clearing browser cache, clearing the cms server page cache, restarting all of the CMS services, opening and closing the page. 

It seems to only happen when they click the "F" Edit via Form red dot, all of the other red dots that allow users to edit individual elements seem to work fine.

Is the reddot custom edit via form reddot?  is the edit via form reddot within a list or dynamic blockmark?
