Pre-Execute code publishing to pages

Normally all goes well and publications happen and pages publish
normally, but about 1 time a month we have a random page from a random
project that gets published without executing any pre-execute code and
thus all the pre-execute code is actually published with the page.

Has anyone seen this happen before.

We are running 9.0.1.28

It is not an issue of our pre-execute code not working.

Randomly on .00000000001% of page publications, the page builder
messes up and does not execute any of our pre-execute code and instead
just publishes it all.
