Reusable, record-specific url link to create a new instance of a repeatable form?

Hello all,

I'm looking for a way to use a reusable url link or QR code to open a new instance of a repeatable form on demand. The smart variable [survey-url:repeating_survey][new-instance] seems like a good start, but I'd like to use it outside of REDCap or REDCap invitations, so it might not be so smart in this case.

The context: clinicians (physiotherapists, doctors,...) who see our project participants in their workplace, at different frequencies and total numbers of visits per participant. We have a short follow-up form that we want them to fill in after each participant visit. It's not realistic to use automatic survey invitations because of the variability of visits schedules (and I don't have these schedules). In addition, I'm trying hard to avoid them having to log into REDCap as users, in order to reduce their workload as much as possible.  
So, can I produce a url to trigger a new instance, which would be specific to the record?
Any idea if this is possible?

NB. The record linked to the clinician who would use the link, not to the study participant. In other words, for this form, the clinician IS the REDCap participant.
