notification of survey that includes PDF

Folks

I am running a project  to collect data on  a case series one of which is a patient outome measurement of function

I would like to send a copy of the PDF of this  specific survey to the treating clinician

by email which is part of their current clinical workflow.

There is provision to send a PDF of the survey to the person completing the survey

There is a notificaton option which does not seem to have the option of including the PDF of the survey.

Can anyone help with how I can send a PDF of  an instrumenr response to a nominated email address on completion

Hi Peter

I haven’t tried it, but I think this should work :

1. In the survey parameters, activate the option "Save a PDF of completed survey response to a File Upload field". I suppose you can even put that field in the same survey (@HIDDEN-SURVEY).
2. Set an alert to be sent to the physician from the "Alerts and Notifications" page. In the body of the message, apply the following that I copied from the piping instruction page :

 "File Upload fields can also utilize the ':link' piping option, such as [my_field:link], where the file's filename will be displayed as a clickable hyperlink for downloading the file, which works on webpages and also inside the body of email text (i.e., survey invitations or Alerts & Notifications)."

In the survey setup, you can turn on the feature to allow subjects to download a copy directly. You can also in survey setup under "send confirmation email" setup, check the box to "include pdf of completed survey as attachment" and it will all happen automatically.

Allow participants to download a PDF of their responses at end of survey?

Display a button for the participant to download a PDF file of their responses for the survey they just completed.

Send confirmation email?

(Email the respondent when they complete the survey)

This method works to attach a PDF to notification, many thanks. This can be sent to any email address.

It is also possible using “survey notifications” from the head of the designer page to arrange to have a notification sent on completion of form with link to the form (via REDCap login) to someone who is registered user for this survey.

In retrospect this option would have worked however the clinical workflow does rely on getting a copy of the PDF questionnaire.

And of course you can specify a send to patient notification with PDF

HI Peter, I do this actually in clinic already, it works really well with a couple of steps. First, is I have a variable where the patient selects the doctor they are seeing, so that I can have only that doctor receive the email. Then in the survey setup, I have the “save a PDF of completed survey response to a file upload field” activated, I tag this variable “@HIDDEN-SURVEY” so they subjects don’t see it. Then under “Alerts & Notifications” I have an alert setup for each doctor, with code to trigger when the survey is saved with complete status and “while the following logic is true:” [doctor]=1 (where [doctor] is my variable where they select their doctor and the number represents the doctor that this alert is for).

 

Then in the alert email body, I select the message attachments and under ”file upload fields”, select the field where the pdf is now saved and the doctor will automatically.
