Multiple recaptcha's on a single page

I'm trying to figure out how to have multiple recaptcha's working on the same wordpress page - see test screenshot below.  I've ruled out the wordpress theme and the Form Maker plugin claims the issue has to be with Recaptcha.  Thoughts??
