1. How many times, in tests, has a team won, when all wickets fell, by
scoring less off the bat than their opponents?

2. How many times, in tests, has a team lost, when all wickets fell,
when its bowlers gave away less runs than their opponent's bowlers?

This has happened only twice. The Extras are given in parentheses.
The sides batting first are listed first.

Aus 586(12) & 166(7) lost to Eng 325(21) & 437(22) by 10 runs
Sydney (1) 1894-95 (Eng followed on)

SAf 169(16) & 239(19) beat Aus 292(6) & 111(3) by 5 runs
Sydney, 1993-94

Also of interest:
Eng 180(18) & 163(20) beat Aus 220(9) & 104(10) by 19 runs
The Oval, 1997

WI 453(4) & 284(23) tied Aus 505(15) & 232(14)
Brisbane, 1960-61

You are leaving out something. Sri Lanka's first innings was 547-8d.
If you include incomplete innings, dozens more Tests will qualify
including some that were lost by an innings.
