Accident Description and Details
Evening of 14th of June, 1986:

Assembly was separated from tack fully occupied

Last train was fishtailing widely and separated from the rail

Loss in coaster speed and passengers were thrown off

Coaster entered third and final loop with not enough speed

Coaster paused at the top and slid backwards

Before the accident:

Roller coaster was shut down twice due to metallic noise coming out of it

Cars were run empty to find source of sound

Nothing was found; maintenance staff decided it was safe to operate

Ride continued until accident occurred

After the accident:

Investigations revealed that issues happened when the operational and maintenance information were translated from German to English

Additional problems in quality control

Manufacturer was bankrupt during ride delivery



Main causation of the accident:

Ways in which each six wheeled car was fastened to axle

Car spun horizontally to a vertical axis, which was affixed to main axle and connected each pair of car assemblies

Cap plate used to join cars to vertical axle above track in ¼” clearance and secured with 4 hex head cap screws

Retaining plate released; cap screws were loose

Cap plate not able to keep car attached to vertical axle

At uplift area, car was derailed throwing victims at height of 25 feet

Inspection held by Alberta province safety inspector one day before accident

Ride is safe of operate

Witnesses mentioned that ride was closed 30 minutes before accident and inspection team were working on derailed cars

Ride had 12 occupants when derailed and 19 people were seriously injured


Investigation Findings

Missing bolts in inside part of left wheel assembly of last train

Cars were only visually inspected

Mindbender crews not supplied with inspection order from General Safety Divisions of Germany

¼ of all axle bolts on cars were loose

Restraint systems were inadequate

No safety checks done by German organization

Every car had 4 bogeys on 2 axles

Anti-roll back system was added

Ride capacity reduced from 16 to 12

Retaining bolt was moved to top of the axle

Maintenance checks and periodic downtimes were increased
