You might want to find _Painting With Light_ John Alton, (1949) New
York, The Macmillan Company.
Although aimed at cinematography the book discusses portrait
lighting for both stills and movies.
John Alton was a well known director of photography in the old days,
about the only one who ever wrote anything about actual lighting
practices. One of his excercizes is learning to light an orange so
that the pores don't show. Previous to about 1965 no one in Hollywood
had pores.
Larger libraries may have the book or be able to get on interlibrary
