The OM-4T is the OM-4Ti. They are one and the same, no difference.
The OM-4 (without T or Ti) is different from the T/Ti (no titanium covers,
and, more important, no full FP-flash synch).
There is a second version of the OM-4 that has improved electronics, but the
two were not distinguished by changing their designation.

None that I've heard of. I think these are just marked-variations.
The big improvement came when the OM-4 was discontinued and
the T (Ti) replaced it with much improved electronics. In fact,
the whole circuitboard was replaced with a completely new one.
Which I seem to recall, introduced some changes to a few of its
functions as well.
