I've not ever done this -- but they covred it on "The pet department" this
week. They recommend TWO people, one on each side. Use cookies, and start by
holding the flap open and teaching her (with cookies) to go through it. Then,
start letting the flap rest on her back as she's going through. Eventually
(over a few days) she'll figure this out. But keep using cookies to reward
her, and only GRADUALLY lower the level to which you hold the flap open.

Two tips, hope they help:
1--Prop it partially open with a stick so that she doesn't see
it as a solid barrier, and so that she gets used to pushing
with her head.
2--Do not let the dog enter or exit the house by any other door
except the flap for a week or two.
