I am surprised your vet did not know what this is. Our vet told us that in
"rumpies" (love the nickname) the absence of the last vertebrae results in
weak sphincter muscles and a protruding anus. Our first "rumpy", Oscar (may
he rest in peace in kitty heaven) had this problem and it was really gross for
the first few months. Not only was there crusting but he often had lumps of
shit sticking out of his butt because the muscles weren't strong enough to
clamp it off. As he got older he had fewer problems with this. Is Emily a
kitten? On the whole it is not something to worry too much about.

When I got my rumpy riser (may she now rest in peace) my vet warned
that becasue the lack of tail is actually a kind of birth defect (malformed
spine resulting in no tail) there can be certain side effects. Some of the
nerves near the base of the tail, especially in rumpies, may also be
malformed and the cat can have problems pooping and other things.
With that in mind, it could be anything. I'm no vet but maybe your manx
has an infection and becasue there might be some nerves missing or
not working, she's not cleaning herself as well and your added attention
helps.
Just an idea.

I've noticed that condition in my cats before, and both were females. Each
one a different breed, but still that "crusty cat butt feature". I don't know
what caused it, but it never really affected either one of my cats. I just
figured they weren't as meticulous about cleaning back there as my male, who
doesn't seem to have it.
