What is the difference between guitars with 22 and 23 frets (besides the
obvious extra note on each string). Is there a difference in
playability?

NOt really, IF you are able to get your fingers to the fret, and don't
have hot dog fingrtips that make playing a not on that top fret
impossible.
Some say that the neck pickup ends up in a less then optimal place when
you go beyond 21 fret necks. Your call.
I have 21 and 22 fret Fenders, and prefer 21 frets, because then I can
tweak the truss rod with out removing the neck. But that's me. I don't
like them high notes anyway.

Well, now, I've never seen a 23-fret guitar. 21, 22, and
24, yes. 21-fret guitars are the original Fenders. Some
one started making a little fingerboard extension on the
necks that came out enough to add a fret, making it 22,
which Fender themselves have been doing for quite a while.
The only real difference there is that you can play one
more note. Now, from 22 to 24 you have to move the pickups
back toward the bridge, which alters the tonality some (too
much for me, although with careful pickup choices this can
be minimized).
This is all Fender history, however; I haven't touched
Gibson (always been traditionally 22-fret on guitars like
the SG, Les Paul, 335, etc.), Rickenbacker (don't know;
never cared enough for the scrawny little neck to try them),
or others.

That kind of depends on the guitar. Just having more frets doesn't mean
it's easy to play the highest frets. My Epi LP has one more fret than
my Strat, but the strat is a lot easier to play at the upper few frets,
because of the way the neck and body join. LPs are much bulkier up
there.

Why do the necks on the SG doubleneck only have 20 frets?
