Once upon a time, songs were not delineated into a clear
antara-mukhada structure. I think Anil Biswas gets credit for
formalizing this structure. (Vish will provide complete pedigree
for this creation, I am sure)
Post Anil-da's in(ter)vention, there were still songs that did not
follow a clear mukha.Da-antara structure. Any ghazal that you
pick will fall into this category lyrics-wise. Some also fall into
this category in terms of the tune employed.
One non-ghazal that falls into this category in terms of tune is
suraiya's "likhane wale ne likh dii merii taqadiir
me.n barabaadii"

This is not exactly in line with what you are asking. ARR has done lot
of experimentation in the structure of the song. A good example could
be Kahana hi kya.
SDB showed shades of this in phoolon ke rang se
Roshan in Swapn zare phoolse
