It contains "Clock" symphony recorded in Oct. 2015.
Fey conducted it ? Or no-conductor ?

I believe Fey had his accident in late 2014. A while later, I read
somewhere that the orchestra had plans to record 101 without conductor
to complete the London cycle; they have been giving concerts with this
piece led by Frieder Bernius around the time you indicate:
The front cover doesn't mention Bernius
<https://www.jpc.de/jpcng/classic/detail/-/hnum/7802181>, but I haven't
seen more details yet to exclude that he (Bernius) conducted it for the
recording. But Fey most certainly didn't.

Is there any indication what sort of accident he had? (I simply read about an accident in his home.

I cannot find anything specific but I seem to recall that is was a fall. Severe head injuries. Probably a "freak accident". It has been around 3 years now, so I am afraid that it was more serious than expected.
In 2015 it apparently was hoped he could return in 2016 but by now it seems unlikely that he will ever be able to conduct again. Admittedly, this is speculation. But if one hears nothing at all about the case or only vague statements of dim hopes, it is usually bad and for reasons of privacy nothing more specific is being published.

Not Fey (out of action since late 2014). Probably either Bernius (who subbed for Fey at a few concerts) or Spillner (or another concert master) leads from the first violin as in the second disc of the latest volume with 6-8,35,46,51.

From Heidelberger Sinfoniker website:
"Leider ist immer noch nicht abzusehen, ob Thomas Fey nach seinem Unfall - vor nun mehr zwei Jahren - wieder auf die Bühne zurück kehren wird. Wir wünschen ihm auch an dieser Stelle viel Kraft auf seinem langen Weg der Genesung."
Basically they are saying that after more than two years since his accident it it still not clear whether he will ever return to the podium again.

If you are asking about how it happened, I don't know either; the effect
was reported to be craniocerebral injury. [Otherwise, I seem to have
read pretty much exactly the same sources that Johannes wrote about.]

He fell down a big flight of stairs in his home, resulting in the head injury.
