I can get it started with a few, some of them borrowed from the Simpsons:

Hugh Jass
Amanda Huggenkiss
Oliver Closof
Phillip McKrevasse
Ivana Tinkle
Adolph Oliver Bush
Mike Rotch
Buster Hymen
Haywood Jablomi

I am looking for a list of these that I know must be out there somewhere...

Three people I've met: Annette Kertin, Joseph King (who refuses to be called
Joe King), and Richard Head (and no one dares call him Dick!).

It's all true, M'lud.

There's a kid here who's first name is pronounced Shu-theed'. It's
spelled Shithead. Also, my wife taught a kid whose name is Lemonjello,
pronounced Luh-mon'-juh-low.

I saw these on the David Letterman show earlier in the week, and thought
it had to be fake, until the people showed their drivers license
immediately after stating their names. Here goes...

(Hope i don't get into copyright trouble.)

10 - Dick Hertz
9 - Phil McCracken
8 - Anita Legg
7 - Harry Dong (obviously Long's brother)
6 - George Titman
5 - Justa Duck (bless her heart, she looked depressed)
4 - Dick Week
3 - Teddy Bair
2 - Harry Bush
1 - Dick Head

A fellow I work with swears he went to school with a girl named
Mary Christmas. There are indeed, people in the Va/NC mountains with
the last name of Christmas.

"During the July 12, 2013, noon newscast, anchor Tori Campbell read a news release that claimed to identify the four pilots of Asiana Airlines Flight 214, which crash landed at San Francisco International Airport on July 6. A summer intern at the National Transportation Safety Board (NTSB) had incorrectly confirmed the spellings of the satirical names to a KTVU staffer who called to verify the release. The names read were: Captain Sum Ting Wong ("something['s] wrong"), Wi Tu Lo ("we [are] too low", referring to the plane's altitude), Ho Lee Fuk ("holy f***", but pronounced by Campbell as /hoʊ liː fuk/) and Bang Ding Ow (onomatopoeia for hitting the ground and exclamations of pain)."
