This is Rotating Quiz #197.

The winner will be the first choice to set RQ 198, in a manner of their choosing.

Please answer based only on your own knowledge; put all of your answers in a single posting, quoting the question before each one.

Answer slates must be posted by 6pm Philadelphia time (GMT-4) on Sunday, 2015-09-27, which gives you 7 days and some number of hours from the time of this posting.

sorry about that. I had this one queued up for a while now, and
thought the order of the answers would allow everyone to at least
guess at the answers.

No, 12. On a question that asks for "name and rank", "lieutenant..."
does not qualify as a "correct answer that does not conform to the theme".
