Teferi's Puzzle Box
Artifact 4
At the beginning of each player's draw step, that player counts the
cards in his or her hand, puts them on the bottom of his or her
library, and then draws that many cards.

Grafted Skullcap
Artifact 4
During your draw phase, draw an additional card. At the end of each
of your turns, discard your hand.

Howling Mine
Artifact 2
At the beginning of each player's draw step, if Howling Mine is
untapped, that player draws an additional card.

If all three of these cards wind up in play at the same time, what
would happen? As near as I can figure, you'd draw an "extra" card for
the Howling Mine, put it and the rest of your hand on the bottom of
your library, draw that number of cards, draw an "additional" card for
the Grafted Skullcap, then make your regular draw. At the end of your
turn you would discard your hand. At the beginning of your next turn,
you would draw an "extra" card for the Howling Mine (which would be
the only card in your hand), put that card on the bottom of your
library, draw one card to satisfy Teferi's Puzzle Box (or would the
Puzzle Box activate first, meaning zero hand=zero action, and then
draw the "extra" card for the Howling Mine?), draw an "additional"
card for Grafted Skullcap, and then make your regular draw. At the
end of your turn you would discard your hand.

So, once all three cards were in play, you would enter your main phase
with three cards in your hand and then discard anything that you don't
play during your main phase and your opponent would do the same. Am I
wrong to assume that if you could get all three cards into play at
once this would stop counterspell-based control decks cold? Not to
mention eliminate any possibility of surprise blockers showing up
(Simian Grunts, etc.) and bring to a screeching halt the current
obssesion with the "Waylay phase"?

GRAFTED SKULLCAP 4 Artifact
At the beginning of your draw step, draw an additional card. / At end of turn,

[It's possible that last should still be "at the end of -your- turn".]

Then you have four things going onto the stack at the beginning of draw step;
in some order, your ordinary draw, and two draw-a-card effects, and the
Puzzle Box effect. (_If_ it's your Grafted Skullcap; if it's not, its effect
doesn't affct you.)

Well, you could do all four in any order.

Again, you could do these four in any order.

(Note: if you -don't- control the Puzzle Box or the Howling Mine, then
the ones you don't control go onto the stack on +top of+ the ones you
-do- control ... so you have to deal with the ones you -don't- control first.
The "in any order" is if you control all three cards...)

Yes.

No; your opponent wouldn't be the controller of Grafted Skullcap, so would
be drawing two cards per draw step (and dealing with the Puzzle Box), and
would _not_ be discarding. Grafted Skullcap only affects "you", its controller.

These are meant to be separate abilities? In that case, on the turn you play
Grafted Skullcap you have to discard your hand, even though you didn't get
to draw an extra card this turn?
I'd assumed the second part was a delayed ability produced by the first, but
if not, the wording they've got there is highly unclear. Perhaps changing
the order the abilities are listed in would be a good idea.
