And the player must have been blinded, or he would have been told what
was going on in a more explicit manner, i.e. "the dwarf's medallion
begins to glow" etc.

Hmm, lessee... You were blind, but had telepathy. The Dwarf was
wearing an Amulet of Life Saving:
blind -> you don't see the amulet glow when you kill the creature
telepathy -> the creature was a "dwarf" not "it"
That's my guess anyway.

I have got a message like that once with an uruk-hai. The answer is:
You were blind (wearing a towel, blindfold, or whatever), and had ESP.
You killed a monster that was wearing an AoLS. Too bad, you didn't see
the amulet so AoLS didn't id for you (it's not always a good idea to get
blind to get ESP work...)
