My 83 Vanagon just started doing the same thing. The transmission
fluid had been leaking, and got down a quart. When I topped up the
transmission fluid, the shifting problem didn't go away.

Virtually everyone with a 1980's vintage VW/Audi with the 3 speed autobox
has experienced that problem if they've owned it long enough. Usually
(hopefully) it's caused by contamination (dirt) in the main valve body
and/or the governor. It can also be caused by hard seals in the clutch
packs in addition to dirt in the valve body. Try having the valve body
disassembled and professionally cleaned by an auto trans shop that is
familiar with Audi/VW's (NOT AAMCO). If that doesn't fix it, stand back
folks, the transmission is coming out! Sorry about that!
Hope for the best, prepare for the worst!
Also, try to minimize revving the engine to redline when its cold unless you
want to go through both the transmission _and_ the engine at the same time.
The transmission will usually shift once the transmission fluid warms up a
little. Manually shifting into 1st ,2nd, and then D will also help coax
that first shift to get you going when cold.
