91 Chev Astro dies on idle - Any ideas?

I've got a 1991 Chevy Astro 4.3L that dies while idling. Any ideas?

It was running with a slight miss before it got warmed up (while in
open loop), then it would run like it was lugging once it got to
overdrive and you try to climb a slight hill or have a head wind. We
changed the distributor w/ coil and ignition module and fuel filter.
That fixed the slight miss when cold. Then it would only miss (badly)
and hesitate after it got warm and went into closed loop.

We changed the ECM (computer) and MAPS (manifold absolute/air pressure
sensor). That cleared up the hesitation and miss at operating temp
while driving.

It would still die at idle, when cold or warm. We saw the spark plugs
looked like they had been run a little lean, so we changed the spark
plugs and distributor cap and rotor.

It dies only at idle and now it dies like it just loses gas - or is
leaned to death - no shudder or anything, just fades out. It runs
great at all other times than idle.

Any help would be greatly appreciated!
