It should be done at 2.4mm or larger, most people find a BCR is fine, though
a curved bar can be used if you think a BCR might be uncomfortable. The
piercing itself is fairly simple to do, but to get 'easy access' to the area
most piercers will do it with you on all fours....so don't be shocked when
they tell you to get on your knees!
Initial healing time is about 2 weeks, full healing in about 6-8 weeks.
Cleaning is probably best done either in the bath or shower, and/or with sea
salt soaks (holding a mug of water over the piercing while sitting on the
toilet works ok)

Has anyone see this before? I've not had a guiche piercing myself, but
I've see a couple and heard of several others, and in each case the guy
has laid on his back ... on all fours isn't one I've come across before.

After my second one as an apprentice, I said "get the fuck"
and made the convincing argument that it's more accurate
to find and mark that "special" spot with the pervert on
his back.

I've done them both ways depending upon the layout of the room I'm working
in. All fours is easier, but if the room isn't set up to work standing at
the end of the table, on the back is doable. It may simply not occur to
some piercers that all-fours is easier, or they may simply not appreciate
the view.
As others have said, it's a standard mark-clamp-pierce procedure like most
others, but you may be on all fours if that's the way your piercer likes to
do them.
You can use a BCR, barbell, curved barbell, but I think the best thing is to
start with a PTFE barbell and then change to the style of jewellery you want
once it's reached the initial healing stage (when it stops weeping/getting
crusty).
Healing time varies so much from person to person, but if you've had a
frenum or sccrotum piercing you can expect the same from a guiche.
Cleaning? Just leave it alone. Take a shower, soap it up, rinse it off.
If the crusties are itching or hurting, soak them and remove them. If not,
leave them alone.
Getting it done is quite undignafied! Mine was done sitting down in (I
think) the Lotus position. Feet together, legs splade, holding me balls
out of the way. Clamped and pierced.
I have heard of people getting it pierced "Dogy Style", although, to me,
the idea of that is horrifying!
Sensation wise, not half as bad as I thought it would be, if memory serves.
I had it done with a ring @ 2.4mm strait off.
Healing was a bit orquard, for a few days after getting it done I was not
running or walking fast anywhere. As a man with chunky legs the ring
ocasionaly was prone to twisting as me legs pased each other. But
eventualy the jewellery works its way into a more comfortable place.
Cleaning wise, I just yanked me jumnk out of the way and used earbuds
soaked in saline to clean of the Lumph and ocasionaly giving a squirt of
Savlon Woned(sp?) wash.
Took a while but it seemed to heal OK for me. Eventualy streched up to 5mm
and its been very happy for the last few years at that size.
